Our role: 24/7 daily care, safety, and coordination. Hospice’s role: medical direction for comfort, nursing/chaplain/social work, medications and equipment related to the terminal diagnosis.

How We Work With Hospice

  • You choose the hospice. We coordinate closely with any Medicare-certified hospice serving Howard County.

     

  • One plan, one team. Shared care plan, regular case conferences, and clear roles to avoid gaps.

     

Comfort first. Symptom relief, calm routines, and gentle, familiar caregiving in private or semi-private rooms.

When It Might Be Time to Consider Hospice

  • Rapid decline in weight, strength, or ability to perform daily activities

  • More time in bed, increasing sleep, or frequent infections

  • Advancing dementia with difficulty eating/drinking despite support

Care & Safety Protocols (Clear & Practical)

  • After-hours protocol: Family calls our on-site team; we triage and contact hospice nurse line as needed.

  • Medications & pharmacy: Hospice supplies comfort meds; we store, track, and administer per orders.

  • Equipment (DME): Hospice arranges delivery/maintenance (bed, oxygen, commode); we set up and monitor safe use.

  • Advance directives: We honor existing directives and can coordinate POLST/MOLST updates with hospice.

  • Transfers/escalation: If symptoms cannot be controlled in place or by family choice, we coordinate with hospice for GIP/inpatient hospice or hospital transfer.

  • Skin integrity & infection control: Scheduled turning/repositioning, moisture management, and nurse reviews.

Who Does What: Hospice vs. Family First Haven

Hospice provides (covered when eligible):

  • RN case management, on-call nurse 24/7
  • Comfort medications & supplies related to the terminal illness
  • DME (hospital bed, oxygen, commode, over-bed table, etc.)
  • Social worker, chaplain, hospice aide, bereavement support

Family First Haven provides (always included in our model):

  • 24/7 caregivers, personal care, hydration/nutrition support
  • Safety oversight, fall-prevention, skin integrity routines
  • Medication administration and prompt symptom reporting
  • Family updates, visit support, and coordination with hospice
